AssureHHI is a leading provider of in-home healthcare with a heart for helping older adults stay independent in their own homes. Since 2003, we have sought to solve the fragmentation in post-acute care delivery that prevents patients from receiving fully coordinated care at home. Far too many are sent home with inadequate care or no support at hospital discharge, only to return to the ER quickly or worse, resulting in hospital readmission. This failure stems from misaligned payment models, poorly coordinated home-based services, and a lack of focus on quality, in short, from a system that doesn’t work for today’s and tomorrow’s patients. It affects our parents, grandparents, friends, and us one day.
It has led Assure to change the face of post-acute and home-based care by Empowering older adults to achieve their goals and provide education and support for their families and community partners. More than ever, patients want to heal at home, and with 10,000 baby boomers turning 65 every day, demand is only growing. Our team of skilled nurses, medical social workers, therapists, and home health aides work with patients and their physicians to achieve their recovery goals while leading the industry in lowering hospital readmissions within the first 30 days after discharge.
